Notes :

1、企业信用等级自评定之日起有效期为三年。

The enterprise credit grade is valid for 3 years satarting from the date of issue.

2、企业信用等级实行复审制度，有效期内，每年复审一次。经复审合格的，加盖复审章后可继续使用；信用状况发生变化的，需重新评定信用等级并更换证书。

The credit grade should be re-examined every year in the period of validity.If the credit status has changed,the credit grade should be re-evaluated and the certificate should be changed.

3、有效期内企业改变名称的，必须持证到发证单位办理变更手续。

If the enterprise changes name in the period of validity,it shall take the certificate to the issue unit to go through the formalities for the change.

4、本证书只证明企业在有效期内的信用状况，不作他用。
